Stimulant laxatives irritate colonic mucosa and promote peristalsis. They may also affect fluid and electrolyte absorption resulting in colonic fluid accumulation. These solutions have been shown to be safe and effective in preparations for colonoscopy, barium xray examinations, and colon surgery. PEG-ELS and SF-ELS are isotonic, poorly absorbed solutions that pass the gastrointestinal tract with no net water or ion absorption or secretion. PEG-ELS and SF-ELS reach a steady-state equilibrium when given at high infusion rates of 1.5 L hour. When PEG-ELS and SF-ELS are used in smaller volumes to treat constipation, the steady state is not reached and the salts are absorbed, negating the safe intent of balanced lavage. PEG 3350 is a chemically inert polymer that does not contain any salt that can be absorbed. It is remarkably non-toxic and can be ingested in large amounts without any harmful effect. PEG 3350 is absorbed only in trace amounts from the gastrointestinal tract. In solution, PEG 3350 binds or sequesters water molecules. These osmotic effects make it an excellent candidate as a new laxative for constipation. Additional options for the management of constipation include colchicines and misoprotil. However, these are associated with side effects. Colchicine is a mucosal poison for gout. Misoprotil is a prostaglandin analog. Both agents are abortigenic and diarrheagenic. Recombinant brain-derived neurotrophic factor and neurotrophin-3 are investigational agents with promise. The 5-HT4 agonists, cisapride, prucalopride, and tegaserod, are effective but face regulatory obstacles to clinical use. Biofeedback and surgery are options for pelvic floor dysfunction. Surgical options for colonic inertia that fail to respond to medical management are also available.

Cell-free supernatant was measured, and the resistance was expressed as the concentration of inhibitor inducing a 50% inhibition of virus growth. As shown in Table I, mutant viruses exhibited a significantly higher resistance to ddI compared with wild-type viruses with a 5-fold increase for L74V and a 9.5-fold increase for K65R and K65R L74V virus. To study the efficacy of colon cleansing regimens, 500 hospital outpatients were randomly assigned to one of four regimens. Two featured castor oil followed by phosphate enema and two used magnesium citrate and bisacodyl. For the 349 patients who completed all components of the regimens, quality of the subsequent barium enema examination was scored by three radiologists in a blind manner. Regimens featuring magnesium citrate and bisacodyl were found to be significantly superior to those featuring castor oil. Calcium bisdioctyl sulfosuccinate did not improve the quality of colon cleansing but appeared to decrease the incidence of abdominal cramping in patients receiving magnesium citrate and bisacodyl. It is hoped that the investigative approach used in this study may serve as a model for future testing of colon cleansing regimens.

In effort to identify marginal predictors of outcome, we retrospectively analyzed 40 cases from our institution in which treatment was essentially uniform 3 ; . The presence of metastases at diagnosis or failure to completely resect the tumor was associated with an extremely poor outcome. Among patients without metastatic disease, a univariate statistical analysis indicated that several clinical and laboratory variables had an adverse impact on outcome. These included age more than 3.5 years, interval of more than 6 months between the first symptoms and diagnosis, urinary excretion of 17-OH 4 mg m-2 day-1, tumor volume 200 cm3, and tumor weight 80 g. Tumor size was highly correlated with a delay in diagnosis and to a lesser degree with older age, indicating that patients diagnosed more than 6 months after the first symptoms and or older than 3.5 years tended to have larger tumors. Multivariate analysis indicated that only tumor size was independently associated with disease-free survival. On the basis of these results, we proposed a set of staging criteria for the classification of childhood ACT Table 3 ; 58 ; .
