Absence of molecular oxygen, however, 'futile cycling' is not possible. The free radical anion can then exert its toxic effects or can be further reduced Figure 5 ; 19, 28 ; . This results in loss of metronidazole resistance under anaerobic conditions. Our observations have important clinical implications. It has been shown that there is a wide variation in MIC for metronidazole both among strains and among resistant strains 31 ; . It was, therefore, suggested that resistance could not be the result of mutations in only one gene, but that mutations in several genes had to be involved 11 ; . In contrast, our data suggest that susceptibility to metronidazole might very well be dependent on the presence of one enzyme only, the rdxA encoded nitroreductase and that the large variation in MIC's can be explained by 'background' variation in the activity of other nitroreductases. This could also explain the lack in reproducibility in MIC when one isolate is repeatedly tested of metronidazole susceptibility, although a strain remains either susceptible or resistant 32 ; . Our present observations suggesting that several other nitroreductases are slowly reducing metronidazole and that the activity of these nitroreductases are regulated by environmental factors may explain these variations in MIC. These variations are, therefore, real variations in nitroreductase activity and not merely methodological artifacts as suggested previously 33 ; . Our study shows that reduction of metronidazole to active components is still possible in resistant strains suggesting that the addition of metronidazole to a regimen may, therefore, increase the efficacy of this regimen in resistant strains. Administration of metronidazole may, therefore, have therapeutic value even in patients infected with a resistant strain. The clinical significance of these observations is probably underestimated and clinical trials are warranted. If this observation proves to be of clinical relevance it may become of great importance as multiple resistant H. pylori strains are an emerging problem. In summary, our data are in concurrence with previous observations showing that resistance is related to mutations in the rdxA gene, encoding for an oxygen-insensitive nitroreductase. In the absence of a functional rdxA gene, however, there is still a slow reduction of metronidazole. These mechanisms become operative in the absence of oxygen when re-oxidation of the metronidazole metabolites becomes impossible. This slow reduction of metronidazole may be responsible for the large 'background' variation in MIC that is observed between and within clinical isolates of H. pylori and may explain why administration of metronidazole may still have therapeutic value in patients infected with a resistant strain. 52 and estramustine.

After the baseline acquisitions, the inotropic state was reduced by an esmolol infusion and increased by a dobutamine infusion, and finally, ischemia was induced in the region under investigation by ligating a distal branch of the circumflex coronary artery. For the esmolol and dobutamine stages, a physiological target was set to a 50% reduction in dP dt and a 100% increase in dP dt relative to baseline for esmolol and dobutamine, respectively. For esmolol, this was achieved at an infusion rate of 100 g kg 1 min 1, whereas the dobutamine infusion rate was 2.5 g kg 1 min 1. In this way, 4 data sets were obtained in each animal.

Property.22 Propranolol induced better LVP recovery in rat ischemic-reperfused hearts than did atenolol, a 1-specific blocker.14 Propranolol caused marked myocardial depression in our model, whereas the same dose of propranolol had no effect on cardiac performance in the rat.14 This may be due to differences in experimental models and species. Esmolol, a 1-specific blocker, has also been reported to exert a cardioprotective effect during myocardial I R. Several studies have suggested that esmolol could be used effectively as an adjunct to cardioplegic solution for induction and maintenance of cardiac arrest during cardiac surgery.7 Modulation of free-radical-mediated reaction and arachidonic acid metabolism11 and inhibition of neutrophil superoxide generation and platelet aggregation12 have been postulated as cellular mechanisms underlying esmolol-induced myocardial protection. While this could be true in vivo, it does not apply to the present study, which was performed in bufferperfused hearts. Further investigation is needed to compare these cellular mechanisms of -adrenoceptor blockers underlying myocardial protection. We investigated the effect of landiolol on cardiac performance during I R. Yasuda et al.8 reported that landiolol has the potential to enhance postischemic cardiac function after warm cardioplegic arrest in isolated rat hearts. They infused a crystalloid cardioplegic solution 37C ; with landiolol 1, 2.5, 5 or 10 mmolL1 ; to the coronary artery and then subjected the heart to 30 min of ischemic arrest followed by 40 min of reperfusion. They observed a significant improvement in postischemic recovery of aortic flow only when 2.5 mmolL1 of landiolol was used. Although their results are in agreement with ours on the cardioprotective effect of landiolol, the effective doses in their study are more than tenfold higher than those used in our study. This may be due to differences in experimental models and species. In the present study, landiolol 100 M ; had a protective effect on the myocardium subjected to I R without causing a reduction in RPP in the pre-ischemic period. These findings suggest that the cardioprotective effect of landiolol cannot be explained only by energysparing mechanisms in the pre-ischemic period. However, landiolol has neither intrinsic sympathomimetic activity nor significant membrane-stabilizing activity.1 In in vitro experiments, landiolol and esmolol failed to inhibit human neutrophil function, including reactive oxygen species production.23 It is not known to what extent the cardioprotective effect of landiolol is due to antioxidation such as that seen with propranolol; further investigation is needed. A limitation of this study is that the doses of blockers were not equipotent with 1-receptor sub.

There are a number of ways for competition authorities to assist IP agencies in taking steps to improve the IP granting process. These include holding interdisciplinary dialogues with patent agencies to encourage greater mutual understanding of each other`s field, commissioning expert reports that study a country`s patent system to determine whether and how it is causing any harm to innovation, and holding seminars or hearings in which academics, public and private sector practitioners, and industry participants come together to discuss problems and possible improvements to IP policies. The effect that competition has on innovation has been studied and debated by economists for many years. Two very different views have emerged. One view holds that large, dominant firms are most likely to produce innovations, whereas the other view is that smaller, more competitive firms are more likely to innovate. Competition agencies tend to endorse the latter view. Unfortunately, neither economic theory nor empirical studies are able to resolve the debate because they both reach mixed results. Theoretical conclusions about the relationship between competition and innovation depend on several factors, such as whether the innovation involves a process or a product, if it is a product innovation ; whether it is geared toward developing a new product or improving an existing one, and how much competition there is in a given market in the first place. Different assumptions about these factors produce different results, with some supporting the idea of highly innovative monopolists and others supporting the idea of highly innovative, vigorously competitive firms. Other theories, however, postulate that the most fertile markets for innovation are those in which the level of competition is neither very low nor very high, but moderate. Early empirical studies supported the theory that moderate levels of market concentration an admittedly imperfect proxy for the level of competition ; produced the most innovation. Those studies were later discounted because they failed to account for certain other explanatory factors. However, more recent studies that do account for additional factors seem to have revived the moderate competition hypothesis. Certain empirical studies done by the OECD take an entirely different approach and search for a correlation between the level of anti-competitive product market regulation and innovation across entire national economies. Interestingly, they find that the relationship is significant and negative, leading to the conclusion that reducing such regulation can pay large dividends. In fact, of the many policy levers studied, reducing anti-competitive regulation was found to be the second most powerful thing that governments could do to raise the level of business R&D spending, and it was six times stronger than enhancing IPRs.
