Direct releases to water or land There are two scenarios in which material could accidentally leave site and directly enter a watercourse or land. Scenario 1: Release of fire fighting water Any fire fighting water used on site would firstly have to fill and overflow the relevant bund s ; , before overflowing onto site. Similarly, firefighting water used on the plant would pool in that area. Then water would enter the site drainage system, fill the relevant sumps interceptor pits and backflow onto site. The site itself is also bunded. For the main site areas substantial amounts of water would have to be used before there is any danger of water flowing offsite. A professional site levels survey has been commissioned to establish to low areas around sites, potential overspill areas and total site retention capacity. Total bund volumes On-site are over 2, 400m3. The site itself can currently hold 192 m3 of water outside bunded areas. In addition drains and sumps around the main plant area will hold another 40m3 before having to pump to the interceptor tank and the sea-outfall pit. The latter hold a joint volume of 43m3. Only when the relevant bund, the relevant surface water sump, its associated drains and the site retention capacity is reached would we require power to pump the water to other bunds, tanks and pits. If power is not available at this stage, the site would then start overflowing onto unmade ground in one of two identified locations. If power is available significantly more storage space in tanks, bunds and the sea-outfall pit is available. Should lab facilities still be available the water would be checked against our discharge limits and if it is within specification be pumped out to sea whilst any foam is retained in the interceptor tanks and the sea outfall pit. Should such water eventually overflow the site lip, its pollutant concentrations would by this time be very dilute. Water overflowing onto unmade ground surrounding the site is unlikely to add significantly to the levels of land contamination already present. There is no vulnerable groundwater aquifer no abstraction points were identified in the vicinity ; . The underlying soil is also of very low hydraulic conductivity. There is no direct access to other drains or to surface water. Due to the clay subsoil, percolation into the subsoil would be slow. Any water pooling in such areas would have to be collected once the emergency situation is remedied and the topsoil in these very localised areas may have to be replaced. SRM do not consider such a scenario to lead to a major accident to the environment.

Editor--Several years ago we reported the drugs that had been most widely sold by value in four countries in Europe France, Germany, Italy, and the United Kingdom ; in 1992.1 We report here the drug sales in the same countries in 1996. We firstly established how many of the 50 products that were most widely sold in the four countries did not have appropriate documentation of efficacy. In France, Germany, and Italy the use of useless drugs had fallen considerably; in Italy this was related to the "cultural change" that occurred in 1994 as a result of a law aimed at rationalising drug classification and reimbursement by the national health service.2 In the United Kingdom the first 50 products by value did not include any drugs considered ineffective. In Italy there was only one drug a preparation of lactobacilli ; in the first 50 whose therapeutic value was unproved, whereas in 1992 there were 10. In Germany there were three products of unproved therapeutic value six in 1992 ; : ginkgo biloba, naloxone-tilidine, and thioctic acid ; . In France there were seven such products 10 in 1992 ; , including herbal extracts, ginkgo biloba, and flavonoids. In addition, the therapeutic classes of the most sold products were strikingly different between the countries, which may partly be explained by therapeutic attitudes and by different means of drug distribution--that is, pharmacies versus hospital. In the United Kingdom, among the first 50 products there were seven glucocorticoids ; for the treatment of asthma, three adrenergic blockers, and three serotonin reuptake inhibitors. In Italy there were two products containing interferon alfa, two benzodiazepines, and two antiandrogens. In Germany the use of contraceptives three products ; and antibiotics predominated. Arterlovenous Fistula of the Lung. Arch. Int. Med. 90: 569 Oct. ; , 1952. An arteriovenous fistula of the lung, successfully removed by surgery, is reported in an asymptomatic 48 year old man. In the differential diagnosis of a nodule in the lung, an arteriovenous fistula should be considered. An arteriovenous fistula may present itself solely as an isolated nodule in the lung parenchyma. Laminagraphic studies are recommended whenever a pulmonary arteriovenous fistula is suspected. Abnormal vessels leading to and from the aneurysm may be identified by this technic. The symptoms and signs of pulmonary arteriovenous fistula vary widely. This applies in particular to facility siting and the choice of distances in a plant between hazardous processes and places of work. The reduction of hazards through siting and spacing is much furthered by property insurance requirements. 26 However, some of the economic gains from inherent safety may go unobserved, which may lead to higher costs and less safety than what would have been achievable. As was observed by Brian Moore of Eli Lilly and Company: "However, the costs associated with an accident or chemical release are typically not included in the economic analysis during the project's design stage. This may result in the analysis providing an incorrect or incomplete result." 27 Hazard indices are important tools in the work for inherent safety in the chemical industry. Several hazard indices have been constructed for judging the inherent safety of a chemical plant. The most widely used of these is the Dow Fire and Explosion Index. These indices serve a useful purpose, but they are highly simplified and there is a large potential for improvement. Hence, all these indices are so simplified that inventories of chemicals are judged by their size, so that a large inventory of dangerous or harmless chemicals are judged the same way. 28 The indices are, generally speaking, less reliable for toxicity assessment than for the assessment of fire and explosion hazards.29 In one study it was shown that the various indices varied so much that there was no similarity in the indices for toxicity, since the toxicity assessment method differed drastically between the models used in the different indices. 30 Some commentators have complained that progress in the actual implementation of inherent safety is too slow. 31 According to Kletz, progress has been real, but the concept has not been adopted nearly as rapidly as quantitative risk assessment, that was introduced into the chemical industry only a few years earlier. 32 One reason for this may be that inherent safety is often difficult but far from impossible 33 ; to implement in existing plants. In conclusion, our data suggest that, depending on the type of cytokines released by resident or recruited cells at early stages of an inflammatory response, NK cells may differentially contribute to the quality and magnitude of innate immune responses 44 47 ; . Many patients combine NHPs with drugs Peng et al. 2004; Smith et al. 2004 ; , and there are risks of interactions Clement et al. 2005; Ernst 2002; Mathews et al. 2003; Peng et al. 2004; Sparreboom et al. 2004 ; . Ernst 2003 ; , in a review of theoretically possible interactions of more than 200 NHPs from acacia to yohimbe ; , concluded that it is hard to conceive of an herbal medicine that has not potential for interaction. Some reported interactions are very serious, but case reports can be misleading as controlled trials for NHPs are scarce Fugh-Berman and Ernst 2001; Mills et al. 2005 ; . There are many confounding factors that can affect how interactions risk prevalence data can be interpreted Bruno and Ellis 2005; McNaughton et al. 2005; Ness et al. 2005; Quandt et al. 2005; Roy-Byrne et al. 2005; Wheaton et al. 2005 ; . Some of the major points to consider include: time span, regional or geographical location, age, health status, ethnicity, and author. Patients might not consider the possibility of risk with self-determined NHP intake during concomitant drug therapy or consumption of other NHPS or dietary items. Most prevalence reports state that patients might not feel the need to tell HCPs that they are consuming NHPs and conversely, that many HCPs do not routinely ask whether patients are ingesting NHPs Giveon et al. 2004 ; . There are a limited number of published clinical cases case reports ; related to NHP-drug interactions. In the case reports examined to establish causality of NHP-drug interactions, it was found that 13% were well-documented, 18% possible, with 69% having insufficient information and were unable to be evaluated Fugh-Berman and Ernst 2001 ; . Of the interactions presented in these case reports, St. John's wort SJW ; 54 cases, 78.7% ; was the most common NHP, with ginkgo 3.7% ; and ginseng 2.8% ; being the next most common. Warfarin 18 cases ; and cyclosporine were the most common drugs involved in NHP-drug interactions. HCPs should note that many drugs, foods, and NHPs may interact with warfarin; and that the International Normalized Ratio INR ; should be checked after a change in medications, drastic change in diet, and after starting or discontinuing phytomedicines. As examples, Dong quai, Danshen and Lycium barbarum increased INR; whereas, green tea and ginseng decreased the INR value. The 2003 Gallup NHP Survey found that 49% of NHP consumers would hesitate to take cert ain herbal NHPs because of insufficient safety information. The survey also showed that 58% of those using NHPs expressed concern about side effects and harmful interactions from taking herbal NHPs. Only 16% described themselves as very knowledgeable about herbs, and 83% indicated that "I'd like to know more about herbal supplements." Another survey showed that few serious problems actually occurred when mixing NHPs and conventional drugs Peng et al. 2004 ; . Variability in response to drug therapy is impacted by genetics, disease and the environment. Interindividual differences in drug disposition also exist and are influenced by age, gender and body mass. Metabolic enzymes and transport proteins are under nuclear regulation.
