Congenital diaphragmatic hernias CDH ; presenting beyond the neonatal period have an extremely varied and misleading presentation. Bochdalek hernia is one of the most important surgically correctable causes of respiratory distress in the child. A recent case of CDH in a previously healthy four year old male child presented with recurrent respiratory infections and gastrointestinal problems. Clinical and radiological diagnostic procedures followed by surgery showed a left Bochdalek hernia without sac. Contents of the hernia included two thirds of the spleen with malrotation of the midgut. Postoperative recovery was uneventful.
