Annually and more frequently after the onset of marrow failure and or development of clonal hematopoiesis. Moreover, the hematologist must be aware of the importance of family genetic counseling and the availability of prenatal diagnosis. However, the hematologist must also be aware that the availability of predictive testing has led to complex ethical issues, such as deliberate conception of a fetus and, more recently, embryo selection for the possibility of providing an unaffected HLA-identical sibling HSC donor. These issues are beyond the scope of this chapter. In summary, a number of obstacles remain preventing the successful use of HCT from alternate donors. However, new treatment protocols are now being evaluated in a cooperative group setting, for the first time allowing new therapies to be tested in larger cohorts of FA patients in a shortened period of time. It is anticipated that new therapies will reduce the risks of alternate donor HCT, making this treatment option more acceptable in the not too distant future.

ABSTRACTS POSTER PRESENTATIONS SATURDAY ; 111 MODALITY SHIFTS IN RENAL REPLACEMENT THERAPY IN CANADA 1990 TO 2003, Yeates k, Trpeski L, Zhu N, Moist L, Morton AR, Kappel J, Fenton SSA, Queen's University, Dept. of Medicine and The Canadian Institute for Health Information The burden of end-stage renal disease ESRD ; and the need for renal replacement therapy RRT ; has steadily increased in Canada since 1990. Studies have reported changes in the incidence of peritoneal dialysis PD ; versus hemodialysis HD ; as a modality of choice in North America. We present data regarding changes in dialysis modality distribution in Canada from 1990 to 2003. Data from the Canadian Organ Replacement Register CORR ; was analyzed using a cohort of incident patients from 1990 to 2003. In this analysis, modality was determined based on that listed for the patient after 90 days on dialysis. The change in the proportion of PD patients with comorbidities such as diabetes and vascular disease were also compared over the same time period. Table 1 describes the proportion of incident patients on peritoneal dialysis in Canada compared to HD from 1994 to 2003. Researchers in CDAP have been involved with several initiatives studying the impact of genes on the risk for alcoholism and its treatment. These studies have been occurring both in humans who have alcohol use disorders and in animal models of alcohol abuse and dependence. Progress is being made toward the ultimate goal of delivering personalized risk assessments and treatments for alcoholism. Dr. Raymond Anton, Director of CDAP and Scientific Director for Clinical Research in the ARC, has been a driving force in the initial efforts of alcohol-related genetic studies in humans. Through collaborations with leading geneticists at Yale University, University of Connecticut, and the National Institutes of Health, several important leads have recently been discovered. One the most promising findings could have immediate clinical application. A small study suggested that a genetic and inherited difference in the mu-opioid brain receptors that bind "natural pleasure molecules" endorphins ; influences how people afflicted by alcohol dependence might respond to naltrexone, one of the few FDA-approved medications available to treat alcohol dependence. In late 2006 at a major national meeting, Dr. Anton reported that the large COMBINE Study, which treated 1383 alcoholics across the United States, found that differences in the mu-opiate receptor gene altered how people respond to naltrexone. Confirming what had been previously suggested, the COMBINE Study findings received significant attention and will likely create considerable excitement in the scientific community. RESULTS To compare the activities of related antimicrobics that vary up to 13% in molecular weight, the results have been expressed as molar concentrations. The salt of the antimicrobic used and the concentration attainable in the serum, expressed in micromoles per milliliter and micrograms per milliliter, are given in Table 2. The morbidity and mortality of serious infections due to Staphylococcus aureus, such as septicemia and endocarditis, remain high despite the availability of antibiotics 7, 12, 13 ; . Before methicillin was introduced, antibiotic combinations were often used to treat such serious staphylococcal infections 13 ; . Since the semisynthetic penicillinase-resistant penicillins and cephalosporins became available, these antibiotics alone have been recommended by many authorities and generally accepted as the therapy of choice for serious infections due to penicillin-resistant S. aureus 3, 4, 6 ; . However, in a major textbook on infectious diseases published recently, the combination of nafcillin and gentamicin was recommended for the therapy of staphylococcal endocarditis 5 ; . We examined malabsorption in AIDS related diarrhea and studied effects of glutamine and alanyl-glutamine on symptoms, absorptive function and antiretroviral drug levels. Randomized, double blinded, placebo controlled study was conducted. Forty-one HIV positive patients with diarrhea and wasting, on ARV drugs were recruited in northeast Brazil. Thirty-three patients on whom ARV levels were determined had malabsorption of one or more drugs. Compared to controls without diarrhea, we found significant malabsorption of ddI p 0.007 ; and 3TC p 0.02 ; . Patients were randomized into four groups to determine the efficacy of glutamine or high or low dose alanine-glutamine versus glycine control. We documented significant improvements in symptom scores with high dose alanine-glutamine p 0.05 and natalizumab.
