METHODS: The study population consisted of 161, 202 users of VHA services in Washington, Oregon, Idaho, and Alaska during fiscal years FY ; 1996 to 1998. We examined both concurrent and predictive model fit for two sequential 12-month periods FY 98 and FY 99 ; with the patient-year as the unit of analysis, using split-half validation. RESULTS: Our results show that the Diagnostic Cost Group Hierarchical Condition Categories model performs best R2 0.45 ; among concurrent cost models, followed by ADG 0.31 ; , RxRisk-V 0.20 ; , and age sex model 0.01 ; . Eight, there is the less obvious, and often ignored, option of delaying the project until a time when economic and financial conditions are such as to provide better assurance that the project will be economically sound. Ninth, there is the "do nothing option". For most projects, there are alternatives already available or some that may become more viable over time. Ferries may be alternatives to bridges over bodies of water. Planes may be alternative to roads, and so on. The economic viability of a specific project will often depend on the future developments of these alternatives. These alternatives may in time compete with the project and reduce its benefits. Or alternatively the new infrastructure may reduce the value of already available options. The possibilities or options listed above should always be considered when deciding on an infrastructure. It is a mistake to assume that a given option especially one determined through a political or even an engineering process ; is the only valid one. Furthermore, when a decision to go ahead with a major project is clearly a political one, made to achieve a political and not necessarily an economic objective, then there is a strong presumption that the project must be covered by ordinary revenue and not by borrowed money. In these circumstances, infrastructure becomes similar to public monuments: they may be necessary to a country but not for economic reasons. When an H&P is completed within the 30 days before admission, the hospital must ensure that an updated medical record entry documenting an examination for any changes in the patient's current condition is completed. The medical H&P or the updated examination must be placed in the patient's medical record within 24 hours after admission, but before a surgical procedure is performed. The final rule applies to all admissions, not just inpatient admissions. The new requirements do not affect a physician's ability to delegate the performance of an H&P to qualified practitioners. The physician, according to the comments, does not have to perform the H&P himself; however, the H&P may be delegated only to those practitioners who have the clinical privileges to perform H&Ps. Ultimately, the physician continues to be responsible for ensuring that the H&P is complete. The new H&P requirements are also reflected in the COP on Medical Record Services. With revisions to the COPs governing Nursing Services and Medical Record Services, the final rule significantly affects the use of verbal orders in the hospital setting. The revised regulations state: "If verbal orders are used, they are to be used infrequently." According to CMS and a number of nationally-recognized reporting organizations, the use of verbal or telephone orders is an error-prone process. The similarity of drug names, according to the National Coordinating Council for Medication Error Reporting and Prevention, accounts for approximately 25 percent of all medication errors. The comments in the final rule provide: "The use of verbal orders should be limited to those situations in which it is impossible or imprac tical for the prescriber to write the order or enter it into a computer."
