By bilateral symmetric thalamic low density lesions on brain imaging and severe neurologic sequelae. There is no information regarding the use of antivirals in these cases but the survey was completed prior to the approval of Tamiflu. Other reports from Japanese authors confirm ongoing increased rates of reporting encephalopathy and encephalitis. More recent reports document a decline in mortality from the mid-1990s to the present. Some reports identify specific patterns of "delirious behavior" and hallucinations in children with influenza. To date, there is no consensus as to why these syndromes of neurologic events seem to be recognized more often in Japanese children than in other populations. Scientific reports originating from the U.S. have appeared as isolated case reports or small case series. The largest of the pediatric series was reported from Houston, Texas, during the early, severe influenza season of 2003-04. In this series, 8 pediatric patients admitted with neurologic symptoms were identified among 478 influenza cases at Texas Children's Hospital. One of these patients was noted to have neurologic sequelae consistent with the ANE syndrome. Only one of the patients in this series received Tamiflu and it was administered after the patient was admitted with neurologic symptoms. Reviewers from ODS and DAVP have requested additional information related to these events from both Hoffman-La Roche and the Japanese regulatory authorities. We have recently received preliminary responses to our requests and this information is currently under review. As part of their response, Roche has provided copies of 2 additional studies that they commissioned to further evaluate safety in pediatric patients. Both the sponsor and the Japanese regulatory authority confirm that Roche, through its Japanese affiliate Chugai, conducted active surveillance in Japan by soliciting AE reports from 70, 000 hospitals, clinics, and physician offices as part of required reporting following the approval of Tamiflu for prophylaxis of influenza. The reports submitted during this active surveillance period are included in the AERS safety database. Both Roche and the Japanese regulatory authority have confirmed that Tamiflu is used extensively among pediatric patients in Japan. Rapid testing for influenza and treatment of illness in pediatric patients are recommended as part of standard health care in Japan and are covered by national health insurance. As compared to the neuropsychiatric AE reports that may be related, in part, to an increased recognition of influenza-associated neurologic events in Japan, the cases of skin hypersensitivity reactions do not appear to be related to a manifestation of influenza illness or increased in a specific population. Pediatric and infectious diseases textbooks do not describe significant dermatologic manifestations of influenza. A review of the scientific literature identified rare case reports of rash associated with influenza. At least one older review of respiratory viral infections in Great Britain noted in graphic format approximately 2-8% incidence of rash with influenza but the review did not describe these findings further. With many drugs, however, skin hypersensitivity reactions are among the most commonly recognized drug-associated adverse reactions and a variety of mechanisms have been described. The important take home message from this is not to allow someone to quit. Knowing that a person with Parkinson's might face any or some of these issues, the challenge is to find an alternative form of exercise that will maintain their interest and allow maintenance of mobility. Overall, the benefits of exercise far outweigh the hazards there are known physiological, psychological and social benefits including keeping the mind more active and lucid for longer. Research is providing evidence of the neuroprotective element to exercise, and so the condition might be maintained for a longer period before deterioration is noted. These might include musculoskeletal strength and power, joint flexibility, postural control, endurance to sustain activity, balance to improve reduce falling, and areas around personal care and domestic activities of daily living. The three activities I can think of that work for all of the components of physiological, psychological and social benefits are brisk walking, swimming and sex. The take home messages are: 1. All people with Parkinson's must be provided with the chance to improve, maintain and minimise the rate of deterioration of mobility; 2. To ensure this happens, referral to the appropriately trained person is essential following diagnosis; and 3. We must educate service providers about the condition so that they can best enable people with Parkinson's to remain mobile for as long as possible. In a recent study by Venuta and colleagues 8 ; of 13 patients who had such valves placed in the most hyperinflated parts of emphysematous lung 11 unilaterally, and 2 bilaterally ; significant improvements in FEV1, residual volume, 6-minute walk distances, and a decrease in the Medical Research Council dyspnea score were observed. This scope has a viewing angle of 30 degrees and a 2.2-m working diameter channel. In this pilot study, 70 patients underwent EBUS-guided biopsies 58 mediastinal, 12 hilar nodes ; . The sensitivity, specificity, and accuracy of the EBUS-guided technique were 95.7%, 100%, and 97.1%, respectively. No complications were noted. Although exciting, this technique requires a dedicated large bronchoscope that is difficult to manipulate and difficult to divert to the aorto-pulmonic window. Peripheral lung lesions are historically difficult to sample by fiberoptic bronchoscopy, and as such represent a challenge for the practicing bronchoscopist. In an attempt to overcome this, an electromagnetic mapping technique to guide the bronchoscopist to the correct subsegments in three dimensions was presented by Becker and coworkers 10 ; in a pilot study of this technique. Electromagnetic navigation using an electromagnetic location board beneath the patient, a retractable sensor probe mounted on a flexible cable, and computer software allowed graphic representation on a monitor to let the bronchoscopist know where his biopsy forceps were in real-time. Using this aid, conclusive biopsies of peripheral lesions were obtained in 69% in 29 patients, comparing favorably to the 30 to 50% yields in simple fluoroscopic guided reports. One pneumothorax and three cases of minor self-limited bleeding occurred. This technique adds considerable time and expense and may not be widely applicable to all bronchoscopy centers.

Sediments represent an important repository of pollutants and a source of contamination for the aquatic food web. Toxicity tests using amphipods as test-organisms have been employed in the assessment of marine and estuarine sediments, together with chemical analyses. The present work aimed to evaluate the quality of sediments from six stations situated in the Santos and So Vicente Estuarine and Harbour System So Paulo - Brazil ; using acute whole sediment toxicity tests with amphipods Tiburonella viscana ; and chemical analyses of metals, PCB's, and PAH's. Other sediment parameters, such as organic carbon and grain size distribution were also analysed. Higher contamination levels were observed in the internal portion of the estuary, where Santos harbour and the industrial zone are located. The toxicity tests showed significant adverse results for most of the samples tested, and the sediments from the internal portion of the estuary presented the highest toxicity. The principal component analyses PCA ; indicated a close relationship between sediment contamination and toxicity. Positive correlation of these factors in the samples studied was used to establish the ranges of the chemical concentrations associated with adverse effects. Such ranges allowed the estimation of preliminary effect threshold values for sediment contamination, by means of multivariate analysis. These suggested values are: Cu, 69.0; Pb, 17.4; Zn, 73.3 mg -1 PAHs, 0.5 mg -1 ; and PCBs, 0.1 g -1 and targretin.
